We are recruiting for an Associate Practitioner who wishes to join #TeamIOWNHS within the Single Point of Access Team. Job Purpose

This role is focussed on supporting people with mental health needs and their carers (friends/relatives/carers) and their supporters in the community enabling and assisting them to meet daily health and wellbeing needs, in line with personal recovery goals.

The post holder will contribute to the ongoing assessment, planning delivery and review of activities and interventions against identified health and wellbeing needs, acting as care/recovery coordinator for an allocated group of service users, in accordance with the agreed personal care and recovery plan.

Job Statement

To undertake the role of Lead Clinician for an allocated caseload of service users under standard care, ongoing and participating in the ongoing assessment, planning, delivering and review of care to meet identified physical, mental health and wellbeing needs. This will include:
