This is an exciting opportunity to join the Central London Community Healthcare Trust Services. We are seeking a motivated, compassionate and caring Rehabilitation Support Worker with experience in therapy or caring for others to apply for this exciting opportunity to work with the Barnet Inpatients and Stroke Services based at Finchley Memorial Hospital.
We are a general rehabilitation ward, and deliver client centred goals which encourages independence for our patients. This job means you will be supporting patients in the hospital, this may also include community settings visits in the borough of Barnet (in their own home or Care Home setting) to aid discharge planning. The Rehabilitation Support Worker will work in collaboration with the inpatient multidisciplinary team completing equipment provision, therapy programs, personal and domestic daily living activities under the guidance and supervision of qualified therapists and nurses.
They will manage a caseload of non-complex cases with some supervision of delegated tasks. They will be carrying out specific Nursing, and therapy activities once training and competencies are completed, to be responsible for collecting a range of information on clients’ needs and health status. The post is well supported by therapy and nurse team leads, and clinicians on the team. There are excellent opportunities for training and continual professional development. There will be other duties such as clerical and administrative tasks as required.
The service operates 7-days a week, which all staff would work shifts as required within weekdays and weekends.
General education of GCSE or equivalent knowledge. NVQ/ diploma level 3 qualifications and other training is desirable.
Must have good literacy and numeracy skills (oral and written communication skills), basic computer and keyboard skills.
Should have experience working in support of therapy or as a carer (in particular falls risk, elderly people with physical or cognitive disabilities) and some experience working in the hospital or community setting. It is desirable that you have access to a car and valid UK licence to drive in the Barnet community to visit patient's homes to support discharge planning.

- To work under the supervision of the multi-disciplinary team, once a therapist/ nurse has made initial contact with the client, you will need to be able to prioritise and manage the clinical responsibilities for a small, varied non-complex caseload, efficiently and effectively use good organisational and time management skills.
- To develop the knowledge, understanding and necessary skills to be able to treat specific, non-complex conditions for eg falls prevention, post fracture and routine hip orthopaedic management, knee replacements.
- To undertake, and report back on delegated clinical nursing and therapy activities that have previously been signed competent, for which adequate training has been given, eg: wound care and bandaging competencies, mobility and falls prevention awareness, equipment provision/ ordering, increasing independence through activities of daily living (ie such as hot drink/ meal preparation, washing, dressing, hygiene/grooming), risk assessments, communication/ speech, social and exercise groups and individual sessions.
- To carry out pre-defined rehabilitation programmes with patients as directed by a qualified therapist for a range of patients, including those with complex presentations, such as mobility and transfers programs and provide suitable equipment as instructed by therapist.
