Job Description

We are looking for an enthusiastic and motivated Physiotherapist to join our Kings Lynn and Coastal Therapy Team. You will be part of a friendly and experienced team of Physiotherapists, Occupational Therapists and Community Assistant Practitioners. There will be an opportunity to work with patients located in community inpatient units and residential homes, as well as providing rehabilitation to patients within their own homes. You will have the opportunity to progress their clinical knowledge and skills through our in-service training program and will participate in regular supervision. Main Duties & Responsibilities 1. Work as an active member of the Physiotherapy team, along with the other members of the Trusts Multidisciplinary team. This may include liaising with other Trusts, statutory and voluntary organisations e.g. GP’s, members of the Biomechanics team, voluntary organisations, carers etc. 2. The role involves lone working and the ability to be able to get yourself from venue to venue as required for clinics, meetings, supervision, assessing and treating patients within community settings. 3. Assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible. 4. Undertake comprehensive assessments of patients with a variety of presentations using clinical reasoning skills. This will include investigative and analytical skills to formulate individual physiotherapy treatment plans / management and evaluation of outcomes. The work will be carried out in the community setting. 5. Work within the integrated team to facilitate early discharge from hospital. 6. Clinical The post holder will: 1. Undertake appropriate Physiotherapy assessments of patients, using investigative, analytical, clinical reasoning skills and outcome measures to inform the development of a graded treatment plan, which may need to be delivered from a range of options. 2. Develop, monitor and review individualised physiotherapy treatment plans of patients, in collaboration with the integrated team, other agencies and the patient, utilising a range of treatment techniques & equipment to maximise their functional ability. 3. Assess patients’ holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan. 4. Where there are barriers to understanding, such as hearing impairment, language barriers, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance. 5. Be required to use tact and persuasive skills in order to gain the patient’s co-operation in their treatment plan. 6. Be aware of assistive technology and refer and utilise as required. 7. Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately. 8. To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework. 9. Ensure own caseload is planned and prioritised according to service targets and patient need. 10. Ensure patient held records are completed for each visit and that documentation is completed within 24 hours. 11. Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual handling and mobilising skills. 12. Contribute to the requisitioning of supplies and equipment through electronic ordering systems. 13. Ensure informed consent is obtained prior to initiating interventions. 14. Could be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies. 15. Be responsible for safe use and maintenance of equipment and supplies. 16. To accept clinical responsibility for a caseload of patients. 1.
