Job Description

This is an exciting opportunity for Support Workers to come and join our newly formed Mental Health Triage and Response Team. This will be a dynamic and responsive team working alongside the existing and Parliamentary award winning Street Car Triage Service. The team will coordinate and manage Section 136 across the Mersey Care footprint. This will include mobilisation and operationalisation of the identified Places of Safety. A responsive and co ordinated approach with emergency services and partner agencies will be essential within this role. The aim of the Mental Health Triage and Response Team is to support service users at the point of crisis and provide expert assessment and coordination. You will ensure that the servicer user receives a high standard of care in an appropriate health setting. In addition you will work alongside Mental Health Practitioners and emergency services to proactively co-ordinate and manage Section 136 across our identified Places of Safety. This will also include working alongside our colleagues from Local Authorities, Acute Trusts , Police, NWAS, BTP and Mental Health Services. The role of the Support Worker within the Mental Health Triage and Response Team will include providing a response to the management of Section 136 and the operating of the identified Places of Safety. You will be the first point of contact and work collaboratively along side the Registered Mental Health Practitioner and also with Police forces, NWAS, Local Authorities, Acute Trust colleagues and Mental Health Services. The Mental Health Triage and Response Team will support service users presenting in crisis and ensure that their needs are met within the identified places of safety and assist with a timely assessment and plan. You will need to demonstrate the ability to effectively communicate, problem solve, recognise risk and be able to effectively support the service user. The role of the Support Worker within the Mental Health Triage and Response Team includes mobilising to support service users from the point of detention under Section 136 through the process of co-ordination, observation, assessment and support within the identified Places of Safety. Support service users whilst in an identified Place of Safety- in line with trust policy and procedures. Participate in regular clinical and managerial supervision as per Trust policy. Participate in supervision to monitor standards of service user care and documentation. Provide information for referrers, service users and carers. Contribute and participate in the implementation of specialist assessments, including risk, with patients requiring crisis care. Support the deliver plans of care as appropriate in collaboration with the multi- disciplinary team Support specialist assessments including risk with patients requiring crisis care. Provide short-term evidence based practices and fixed term interventions. Support the delivery of education and self-help materials within a variety of locations. Develop and maintain effective working partnerships with services in other statutory and non-statutory organisations. Support the implementation of integrated care pathways, including appropriate service user assessments/referrals and models of care. Ensure the clinical environment is conductive to promote service users health, safety and well-being, thereby preserving and respecting each services user’s personal dignity, privacy, religious and cultural beliefs. Ensure record keeping is in accordance with Professional and Trust standards. Work in a professional and collaborative manner with the Multi- disciplinary Team. Participate and promote positive intra-agency working. Participate and assist in setting standards of service user care based on sound research findings and evidenced based practice.
